Dr. Karen Horton and our team are pleased to announce that our Plastic Surgery practice has made the transition to fully digital charts. We now use Symplast to keep patient records and make appointments with us.

You may already have received an email request to complete forms for “San Francisco Plastic Surgery”, our official practice name, if you have an upcoming appointment. In this email, you will receive a link to download the Symplast app as well as a unique registration code.

Even if you are an established patient of Dr. Horton’s or Emily Sespaniak, NP’s, completing these forms online through the MozartMD site will assist with creation of your profile in our new system. And don’t worry: all patient information is fully encrypted so your personal data remains safe.

Eventually, you will have the opportunity to view your own before and after images once we are up and running with Symplast!
