Before and after bilateral DIEP flap breast reconstruction in a 50 year old breast cancer survivor. She had undergone a right mastectomy without reconstruction, and had a past submuscular left breast augmentation with the Snoopy deformity (related to an abnormally high position of her implant under the muscle).

She had recovered from chemotherapy and radiation to the right chest, and wished to have a left prophylactic mastectomy and DIEP flap reconstructions on both sides. Her left implant was removed at the time of mastectomy, and her left pectoralis major muscle was repaired to the chest wall.

A right-sided delayed DIEP flap was performed with replacement of missing chest wall skin with skin from her abdomen. A left-sided immediate DIEP flap reconstruction was hidden beneath her own breast skin with removal of excess droopy skin and a lift at the same time.

Secondary nipple and areola complex reconstructions were performed as an outpatient procedure. Follow up images are shown at one year after her DIEP flaps. Note the low position of her abdominal scar and results that are – and should be – equivalent to a cosmetic abdominoplasty!
