Before and after bilateral breast revision with liposuction of the axillary rolls in a 58-year-old woman. Her submuscular implants felt tight and created pain and deformity when she contracted her pectoral muscles, particularly on her right dominant side.
She wanted better symmetry to her breasts and more physical comfort. Her revision involved removal of her old implants, replacing her pectoralis major muscles to the chest wall, and creation of a new implant pocket on top of the muscle. New smooth round silicone gel breast implants were placed in the prepectoral/subglandular position.
Her nipples and areolas were made more symmetric though a breast lift, which created a new lollipop scar on each breast. Liposuction removed excess fat from her armpits and bra rolls.
Long-term follow up photos are shown 9 years after surgery. Her symmetry has persisted even through body changes as she has aged. She will now follow up every five years and is relieved to know that she does not need any additional surgeries – only if she has problems or wants her implants out.
