Before and after bilateral breast implant revision in a 68-year-old woman. She had a previous breast augmentation several years ago, and after losing some weight, she decided that she wanted to have slightly larger implants to create additional upper pole fullness that she had lost.

She was delighted that she could participate in sizing her new breast implants! An in-office, awake procedure was planned under local anesthesia, an oral anti-anxiety medication and inhaled laughing gas. Local anesthesia was injected into her old inframammary fold scar, the old implants were removed, and the inside pocket was inspected.

Temporary saline-filled sizers were filled to her previous breast size, then gradually inflated until the patient’s desired result was achieved. She was able to sit up during surgery and observe different implant volumes via a mirror in the operating room – she decided on an implant a few sizes larger than her previous ones.

Following irrigation of the pocket and the implant with an antibacterial solution, the new implants were inserted through the existing incision under the breast. No drains were required since no internal incisions such as for a capsulectomy were needed. After surgery, she showered fully in two days, and resumed exercise in two weeks.

Follow up photos are shown 5 weeks after surgery. She loves her results and even more that she got to choose them, personally!
