Before and after bilateral nipple-sparing mastectomies and prepectoral implant reconstruction in a 25-year-old BRCA gene-positive woman with a high risk of breast cancer development. Her family members who had developed breast cancer all died from their disease in their early thirties. She wanted to avoid this fate!

A combined oncologic and reconstructive procedure was planned through incisions hidden under her breasts. Smooth round permanent but postoperatively adjustable saline implants were placed on top of the muscle, in the prepectoral position. After surgery, the volume was adjusted twice as surgical swelling went down.

Follow up photos are shown 3 months after surgery. She is ready for port removal and likely implant exchange to silicone, now that she is sure about her chosen implant size. Silicone implants will have less rippling than her current saline implants. This final procedure will be performed under local anesthesia since she does not have any capsular contracture or other necessary reason to go to sleep!
