Before and after breast revision in a 64-year-old breast cancer survivor. She previously had a left skin-sparing mastectomy and submuscular implant reconstruction 20 years ago, and a right nipple-sparing mastectomy with prepectoral implant reconstruction several years after that.
Her left submuscular implant created a significant animation deformity and had stretching of the lower pole, causing a baggy breast reconstruction at the bottom. The right nipple and areola sat lower than the left reconstructed nipple, creating asymmetry.
Her revision involved replacement of the left pectoralis major muscle to the chest wall and insertion of a new silicone gel breast implant on top of the muscle, in the prepectoral position. A new incision was made in her inframammary fold to reduce the excess lower pole skin.
The right breast implant was found to be ruptured and was also exchanged for a newer device. Liposuction removed excess fat from both armpits (axillary rolls). A right balancing breast lift helped to create improved symmetry to her breasts. Medical tattoos brightened both areola circles and created better symmetry.
Follow up photos are shown 4 months after surgery.
