Before and after bilateral prophylactic nipple-sparing mastectomies and immediate implant reconstruction in a 29 year old woman with a genetic predisposition to developing early breast cancer. She had always considered breast augmentation. She was delighted that she could achieve two goals with one surgery: risk reduction and fuller breasts!
Her breast tissue was removed through incisions hidden in her breast folds. Smooth round permanent postoperative adjustable saline-filled breast implants were placed on top of the muscle, in the prepectoral plane. The implants were filled to the same volume as her breasts on the operating room table. Liposuction removed excess fat in her armpit region.
After surgery, she chose to have the adjustable implants inflated further with two additional saline injections until she finally fit the bra she had always wanted to wear. She returned to work and exercise without restrictions at 6 weeks.
Follow up photos are shown 3 months after surgery. She is planning to have her implant ports removed and switch her implants to smooth round silicone gel breast implants. The sole advantage of saline implants is that they are adjustable – this is why they are used for breast reconstruction, as our patients can participate in breast sizing in their own bodies! Silicone gel implants will look and feel more natural once she has decided on her final volume.
