Before and after right breast revision in a 37 year old woman with previously placed breast implants.  She presented with a sudden swelling of her right breast.  She had not had any trauma or injury to her chest.

An ultrasound performed elsewhere revealed blood around her breast implant.  She was taken to the operating room and a hematoma (blood collection around the implant) was evacuated.  Her implant was changed and she had a drain in place for two weeks.

She was referred to a Hematologist who performed a workup for a bleeding disorder, which she indeed had!  By caring for the whole patient, taking a careful history (and learning this had happened once before, soon after her surgery in another state) and determining that excessive Excedrin use (which contains aspirin) led up to her spontaneous hematoma, we not only fixed the acute problem but prevented future bleeds and possibly saved her life!  If she had fallen and hit her head, the results could have been disastrous.

Follow up photos are shown 6 months after hematoma evacuation, capsulectomy and implant exchange.  She now knows that she needs to stay away from anti-inflammatories and that she is at potential bleeding risk in the future if she takes these types of medications.  Knowledge is power!
