Before and after bilateral breast revision in a 61 year old woman with a past implant reconstruction.  She had undergone bilateral skin sparing mastectomies and tissue expanders, which were exchanged for saline implants.  She felt her implants were too small for her frame, and she disliked the animation deformity that resulted every time she flexed her chest muscles.

Her breast revision involved reopening her old mastectomy scars, removing the old implants, separating her pectoralis major muscles from the undersurface of her breast skin, and reconstructing her chest wall anatomy.  The original boundaries of her breast footprint were recreated to restore a more natural and proportional breast form.

Smooth round cohesive silicone gel breast implants were placed on top of the muscle, in the subcutaneous or prepectoral position.  She took zero pain medication after surgery, which was a huge change from her painful tissue expansion!

Follow up photos are shown 6 weeks after surgery.  She has resumed exercise and feels whole again.  We will plan nipple and areola reconstruction as an office procedure under local anesthesia at the 6 month mark to complete her reconstruction.
