49-year-old woman with a history of left breast cancer, a near-radical mastectomy, chemotherapy and radiation without reconstruction. She was an excellent candidate for a DIEP flap. Her personal goals were to be reconstructed larger than her natural breast size. A large DIEP flap on the left side and a balancing breast augmentation on the right achieved her goals and created a natural and aesthetic result. Postoperative images after nipple and areola reconstruction using a free nipple graft and medical tattoo are shown.
