46-year-old BRCA gene-positive woman with a history of bilateral prophylactic mastectomies to reduce her risk of breast cancer and submuscular implant reconstructions that were plagued with recurrent capsular contracture, pain and breast asymmetry. She wished to have conversion of her implant-based breast reconstructions to flap reconstructions using her tummy tissue. Both implants were removed, capsulectomies were performed (removal of scar tissue from around the implants), her pectoralis major muscles were returned to the chest wall and reconstructed, and bilateral DIEP flaps were performed in a single operation. She achieved her goals of improved breast symmetry and aesthetics, a soft and natural feel and improvement of her abdominal contour.
