60-year-old woman with a history of left breast cancer and mastectomy, chemotherapy, radiation and a submuscular tissue expander reconstruction. She was unsatisfied with her implant reconstruction and wished to have a flap reconstruction bilaterally. Her left implant was removed, the left pectoralis major muscle was returned to the chest wall and reconstructed, and a right prophylactic nipple-sparing mastectomy was planned. Bilateral DIEP flaps were performed at the same procedure to achieve multiple goals in one surgery. Postoperative images at one year after her left nipple and areola reconstruction are complete are shown. Note that her right breast reconstruction appears completely natural, and the left breast reconstruction is significantly improved.
