46-year-old woman with right recurrent breast cancer after past lumpectomy and radiation. A bilateral nipple-sparing mastectomy and immediate reconstruction using the DIEP flap was performed, incorporating a breast lift and bilateral free nipple and areola grafts into the procedure. Better symmetry was achieved in a single surgery. Follow up photos at 6 months are shown, with slight loss of pigmentation of her areolas that were later tattooed to fill in light areas.
