44-year-old woman with right breast cancer who wished to also have a breast reduction. A right nipple-sparing mastectomy designed using a breast reduction pattern and immediate right breast reconstruction using the DIEP flap was performed. A left breast reduction was carried out at the same time to achieve symmetry in one operation. Her right nipple and areola was reapplied to the right breast reconstruction as a free graft so that no additional surgeries were needed. Postoperative images at 9 months after surgery and following chemotherapy are shown.
