Before and after bilateral breast revision in a 34 year old woman. She had previously undergone two cosmetic procedures and was still not happy with her results. She had a severe animation deformity (motion or flexion deformity) from the submuscular placement of her breast implants, and a double bubble deformity that was apparent on pectoralis muscle flexion.

This patient especially disliked the wide distance of her breast implants – also related to them being under the muscle. The muscle vector pulls the implants up and out, following the muscle action vector. She hoped for more natural looking cleavage, which is what subglandular breast augmentation provides.

Her revision involved removal and replacement of her implants, replacement of her pectoralis major muscles to the chest wall and creation of a new subglandular pocket. This simple part of the revision eliminates all deformities related to implants being placed under the muscle. New smooth round silicone gel breast implants were placed on top of the muscle. Liposuction of the axillary rolls permanently removed armpit fat to complete her procedure.

Follow up photos are shown one month after surgery. Her bruising is nearly all gone from the liposuction. She is very excited to resume weight lifting and get on with her life without the double bubble and ugly chest muscle flexion “party trick”!
