Before and after bilateral breast revision in a 53 year old breast cancer survivor. She previously had undergone bilateral skin sparing mastectomies and partially submuscular implants, resulting in a significant animation deformity with pectoralis muscle contraction.
Her axillary rolls were never addressed, creating a relative deformity of the excess fat in this area. She also had undergone nipple and areola reconstruction with a flat nipple and faded areola tattoo. Her goal was to have more normal looking nipples and areolas but not to have much more nipple projection.
Her revision included conversion of her implants from under the muscle to on top (prepectoral reconstruction). This was done using the existing scars under her breasts. New silicone gel breast implants were placed on top of the muscle. The tight bands on the sides of her breasts were naturally released and relaxed after putting her chest muscle back down on the chest wall – where it belongs!
Liposuction permanently removed excess fat from her armpit region (axillary rolls) and the back fat/bra rolls. Her nipples and areolas were reconstructed by redoing the medical tattoo to create definition of the central nipple circle and a brighter, smaller areola circle surrounding the nipple.
Note the brighter areola takes attention away from the previous faded circle, and how removal of the armpit fat creates a much more aesthetically pleasing result! Note also the obvious loss of her animation deformity with pectoralis major muscle flexion. Follow up photos are shown one month after surgery.
