Before and after breast reduction in a 19 year old patient. This patient identified as male and was starting his journey in a female to male (FTM) transition. Although I don’t do a lot of transgender surgery, I do several breast reductions each week! (I stopped counting at 4000 breast reductions…)

This female to male transgender patient who was beginning their surgical journey and had recently begin testosterone replacement. He had obviously large and pendulous breasts and wanted a significant reduction that would be further helped by male hormone replacement.

A breast reduction was performed though the usual “superomedial pedicle” technique, with the objective of a very small and relatively flat breast afterward. The blood supply to the nipple is maintained through a connection of tissue from the upper and inner part of the breast. This preserves the very best sensation and ability to breast feed (in non-trans patients).

Follow up photos are shown 8 months after surgery. This patient’s scars are maturing, and they are very pleased with their results. They have continued testosterone replacement, as evidenced by the chest acne. They feel their chest matches the rest of their self-image and are very happy with their decision!
