Before and after bilateral breast revision in a 42-year-old woman with mildly tubular-shaped breasts. Her previously placed implants under the muscle, demonstrated deformity and asymmetry. The right implant had healed too high due to its submuscular placement, creating a lower pole deformity. Her implants were also hard and uncomfortable – this condition is classified as Grade IV capsular contracture.
This revision involved removal of the old saline breast implants, replacing the pectoralis major muscles back to the chest wall and insertion of new smooth round silicone gel breast implants on top of the muscle, in the prepectoral position. The subglandular position allowed the implants to sit lower in the breast pocket, creating a more natural lower pole and correcting the tubular breast shape.
The nipples and areolas were lifted by a mastopexy procedure. Liposuction removed excess fat from the armpits and bra rolls to complete the revision.
Follow up photos are shown 6 weeks after surgery. She is ready to begin scar therapy and is already incredibly happy with her result!
