Before and after bilateral tubular breast augmentation in a 33-year-old woman. She had true “hypomastia” or hypoplasia of her breast tissue – her chest almost looked like a male chest due to lack of breast development. She was not transgender. She was worked up by her family doctor and gynecologist to ensure her hormone levels were normal prior to surgery in case hormone replacement would also help with breast development.

She wanted to have fuller breasts like she had been creating since puberty with padding in her bras. Smooth round silicone gel breast implants were placed on top of the pectoralis major muscle, in the subglandular position using an incision in the fold of the breast.

Tissue expanders were not required. The breast is a naturally distensible organ – think about how much breasts can grow during lactation and breastfeeding. On the operating room table, intraoperative sizing with a saline-filled sizer allowed the breast to expand and accommodate an implant.

Follow up photos are shown 2 years after surgery. She finally feels “normal” and loves how her breasts are in proportion to her body!
