Before and after breast implant revision in a 36 year old woman with tubular breasts. She previously had a breast augmentation in the submuscular plane. She disliked the shape of her breasts and felt like they looked worse than before. She also had a significant animation deformity from the submuscular placement of her implants, and a “double bubble” from the implant sitting lower than her natural fold and failure to correct her “old fold”.
Unfortunately, breast implants placed under the muscle accentuate a tubular breast deformity – which consists of a high, tight inframammary fold, a cone-shaped breast and wide space between the breasts. Proper tubular breast correction ideally involves subglandular placement of a silicone gel implant (placed on top of the muscle) to help redistribute a conically shaped tubular breast on top of a round implant. The implant helps to recruit lower chest wall skin and form a more round base to the breast, more cleavage and an overall more aesthetically pleasing breast.
For this patient’s revision, her pectoralis major muscles were returned to her chest wall and a new smooth round breast implant was placed on top of the muscle in a new subglandular pocket. The double bubble and animation deformity are eliminated by placing implants in the subglandular position.
Follow up photos are shown 6 weeks after surgery.
