Before and after breast revision in a 66 year old woman. She had previously placed saline breast implants several decades ago. The right implant had deflated and the left breast implant was bottoming out, creating a higher than normal position of her left nipple and a low-lying implant on the chest wall.
Both implants were removed and replaced with smooth round silicone gel breast implants. Her left inframammary fold was reconstructed, and scar tissue was removed at the top of her pockets to allow the implants to sit higher on her chest.
Follow up photos are shown three months after surgery. She returned to her home state across the country, happy that her left fold is holding up well at this point in time. She is well aware that correction of bottoming out usually involves overcorrection of the fold, allowing for tissue relaxation over the first year after surgery.
She is also happy to know that this will likely be her last surgery, ever! Implants do not need to be removed and replaced every ten years. Additional surgery will only be required if she is having a problem or wishes a change, such as implant removal or a change in implant size.
