Before and after bilateral breast reconstruction revision in a 64-year-old breast cancer survivor. Her past cancer history included several lumpectomies, bilateral mastectomies, chemotherapy, left chest radiation, a right Latissimus Dorsi (LD) flap reconstruction, and a left TRAM flap reconstruction with placement of implants under both flaps.

She was hoping for a smaller breast size, improved symmetry and more defined nipples and areolas. Her revision involved removal of her previously placed breast implants, capsulectomy to remove scar tissue, and insertion of new smooth round silicone gel breast implants on top of the chest muscles, in the prepectoral position. Her inframammary folds were reconstructed using suture techniques to provide better definition to the lower pole of her breasts.

Liposuction removed excess fat from her armpits and bra rolls – this area is called the axillary rolls. Her past right nipple reconstruction, which was placed extremely far laterally (to the outside) was relocated as a free nipple graft to a more normal position on the breast. Medical tattoo brightened her past nipple and areola reconstructions and took attention away from the past abnormally shaped areola tattoo.
