Before and after bilateral explantation in a 32-year-old woman. She had developed tight, uncomfortable capsular contracture and asymmetry following a right breast infection the previous year and wanted her implants out after having them for fifteen years.

Explantation was performed in the office under local anesthesia. Removal of the internal scar tissue (called a capsulectomy) is really only necessary if leaving it in place would cause deformity or if there is a cancer concern.

In this case, removal of the breast implants was all she required – the scar tissue slowly faded and softened, obviating the need for a capsulectomy. After surgery, she took two weeks off exercise and noticed the firm scar tissue on the left started to soften and go away within days, with no palpable capsule left at two months.

Follow up photos are shown 1 year after surgery. Her breast skin contracted to assume her preoperative form, without any loose or baggy breast skin. She is very satisfied with her decision to have her breast implants removed – “no regrets!”
