Before and after bilateral breast revision in a 58-year-old woman with painful, contracted breast implants and mildly droopy (ptotic) breasts with a low nipple position and elongated breast shape.  She avoided lying on her stomach in yoga classes due to pain from the hard implants and at the beach, had to dig out extra sand to accommodate her Grade IV capsular contracted implants.

Some Plastic Surgeons call these types of contracted breasts “rocks in socks.”  We aim not to disparage women who are seeking a more natural look to their breasts and avoid labeling them with a particular “deformity,” instead educating them about their surgical options for a breast revision.

This patient wanted her breasts “to look to more normal,” to feel softer and be rounder.  She also wanted her nipples to sit higher up on her breasts and was willing to accept a new lollipop scar that is necessary when lifting the nipples.

Her revision involved removal of the old breast implants and opening up a new pocket for the breast implant (a capsulectomy).  New smooth round silicone gel breast implants were placed on top of the muscle, in the subglandular position.  A mastopexy (breast lift) allowed her nipples to be raised, and it removed excess lower pole baggy skin.

Liposuction removed excess fat from her bra rolls and armpits – the axillary rolls.  Follow up photos are shown 6 months after surgery.  She is actively doing scar therapy and is very pleased with her fuller, perkier, more youthful looking breasts!
