Before and after bilateral breast implant revision and liposuction of the axillary rolls in a 32-year-old woman.  At the same time as a liposuction procedure elsewhere on her body, she wanted to improve the shape and contour of her breasts, and to be slightly larger in size.

Her old implants were textured silicone and completely under the muscle, causing a significant animation deformity when she flexed her pectoralis major muscles.  For this surgery, her old scars under the breasts were used to remove the old implants and separate her chest muscles from the undersurface of her breasts.  The pectoralis muscles were replaced to the chest wall and their attachments were reconstructed back to the ribs and the sternum.

New smooth round silicone gel breast implants that were larger in volume and had a higher projection were inserted on top of the muscle in a new subglandular pocket.  Simply converting the implants from under the muscle to on top of the muscle eliminated the animation deformity and created a rounder lower pole and natural shape overall to the breast.

Liposuction permanently removed fat from her armpits and bra rolls through two tiny incisions on each side of the breasts.  After surgery, she took a full month off exercise and allowed her muscles to heal in their new position on the chest wall.    

Follow up photos are shown 6 months after surgery.  She felt so good about her breasts that she began exercising and eating more healthily, losing some weight which accentuated her liposuction results!
