Before and after bilateral breast augmentation revision in a 66-year-old woman. Her nearly thirty year-old saline breast implants that were under the muscle were tight, causing tension and a pulling sensation whenever she used her arms.
She disliked that she did not have normal cleavage unless she wore a bra or sports bra to push her breasts together. She wanted to keep implants but to have a mor natural looking result. As a former body builder, it is surprising that her implants were ever inserted under her chest muscles!
Her revision involved removal of her old saline implants, replacing her pectoralis major muscles back to the chest wall and reconstructing their attachments to the ribs and sternum, and insertion of new smooth round silicone gel breast implants on top of the muscle, in the prepectoral or subglandular position.
Follow up photos are shown 1 year after surgery. She is very pleased with her cleavage – now that her breasts are closer together without needing a bra to push them together – and will follow up with us yearly going forward.
