Before and after bilateral breast implant revision in a 32-year-old woman with previously placed submuscular saline breast implants. She reported that “they never dropped,” which is obvious by her superior (high) and lateral (wide) malposition of her implants. When implants are placed under the muscle, the muscle pulls them up and out along the natural pull of the muscle, causing them to feel tight and sit abnormally high and wide.
She also had a severe animation deformity evident when she flexed her chest muscles. She wanted her breasts to be larger and more natural looking, with better cleavage. Her revision involved removal of the old saline breast implants, replacing her pectoralis major muscles to the chest wall, and creation of a new subglandular pocket on top of the muscle. Larger smooth round silicone gel breast implants were placed in this new pocket. Liposuction removed excess fat from her axillary rolls (the armpits and bra rolls).
Follow up photos are shown 3 months after surgery. Her animation deformity has been corrected, her implants are now seated behind the breast tissue and nipple, and she feels voluptuous and beautiful!
