Before and after bilateral explantation in a 49-year-old woman with contracted breast implants that were inserted nearly twenty years ago. She felt pain nearly every day and no longer wanted them in her body.
Explantation was performed in the office under local anesthesia using her existing infra-areolar scar. We usually recommend making a new incision in the inframammary fold, but she had a history of making hypertrophic scars that took years to fade. In this case, it was reasonable to use her existing incision rather than make a second incision that might heal with another unsightly and symptomatic scar.
She began scar therapy one month after implant removal, and thankfully she healed well this time. Follow up photos are shown 6 months after surgery.
