Before and after bilateral breast revision in a 66 year old breast cancer survivor. She previously had bilateral mastectomies and reconstruction in another state. Her implants were partially under the muscle, as seen by her severe animation deformity created on pectoralis major muscle flexion. She had stopped exercising due to her muscles jumping so severely and was told by local surgeons that no additional improvements were possible.
When a patient comes for an assessment of their breasts, we discuss in detail the things they don’t like and go over what can be done to MAKE THINGS BETTER. There is usually SOMETHING we can offer to make the results more symmetric, less uncomfortable and better overall!
In this case, both implants were removed from under the muscle and the pectoralis muscles were returned to the chest wall, reconstructing her chest wall anatomy. Stiff, tight old dermal matrix that was contributing to the deformity was painstakingly removed from under her breast skin.
Liposuction of her axillary rolls and the unusual fat roll at the top of her chest smoothed out this area and improved the appearance of her upper pole. Smooth round silicone gel breast implants were placed on top of the muscle, in the prepectoral position.
Follow up photos are shown just two weeks postoperatively. Her breast drains were in place for 10 days. Her bruising is beginning to fade.
She has started implant massage and will be flying home. We surpassed her expectations! She looks forward to getting back to exercise in another few weeks and visiting us in San Francisco for her three month follow up.
