Before and after bilateral breast revision in a 43 year old woman with the BRCA gene. Her completely submuscular breast reconstructions after bilateral prophylactic mastectomies caused pain, tightness and an inability to exercise the way she wanted. Her implants were too narrow for her breast footprint, contributing to a less-than-satisfactory aesthetic.
She was thrilled to find a surgeon who felt comfortable changing her implants from under the muscle to on top of the muscle. She never developed breast cancer, and therefore did not have radiation damage to her tissues – so there was no reason to deny her a revision.
Through the scar in her inframammary folds, the old implants were removed and the pectoralis major muscle was returned to the chest wall. The last image shows intraoperative details of muscle repair and making a new pocket in the prepectoral space. New smooth round silicone gel breast implants were placed in this new plane, relieving her of her animation deformity and symptoms from submuscular implants.
Follow up photos are shown 10 days after surgery.
