Before and after breast implant revision in a 53 year old woman who had countless past breast surgeries. She wanted to downsize her implants as a final surgery but did not have time to commit to a long recovery. Since her pocket did not need a major change, she was a candidate for in-office implant exchange.
Under local anesthesia, her existing inframammary fold scar was opened, her old implants removed, and a temporary sizer was inserted. The patient was allowed to choose her new implant size by sitting up and looking in a mirror (with our input, of course). She selected lower profile smooth round silicone gel breast implants that were 75 cc smaller than her current implant size.
After washing out the implant pocket with an antimicrobial solution, the new implant was inserted with a Keller funnel, which helps to prevent skin bacteria from entering the pocket. After surgery, she was allowed to shower in two days and resume exercise in two weeks.
Follow up photos are shown 6 months after surgery. She is overjoyed that she was able to choose her own breasts after several past revisions that she was never satisfied with.
