Before and after bilateral implant reconstruction revision in a 57 year old woman with the BRCA gene. She previously had undergone bilateral nipple sparing mastectomies and implant reconstruction.

She had healed with severely hypertrophic scars and breast asymmetry. One implant pocket was too big, the implant was too small and it slid around freely in the pocket. The other pocket was too small, with capsular contracture. 

Her revision involved bilateral capsulectomy and implant exchange. Intermediate stiffness/cohesiveness smooth round silicone gel breast implants were placed on top of the muscle, in the prepectoral position. Liposuction debulked fat in her armpit region and fat grafting added a small amount of fat to her thin mastectomy skin flaps. 

Follow up photos are shown 6 months after surgery. She has improved symmetry and her scars are maturing well so far, without recurrence of hypertrophic scars. Although we can never reestablish perfect symmetry and the look of natural breasts after mastectomies, we aim to achieve symmetry, balance and beauty for every single breast reconstruction.
