Before and after bilateral breast implant reconstruction revision in a 60 year old breast cancer survivor. Over 20 years ago, she had skin sparing mastectomies and submuscular implant reconstructions. She had chronic tightness and discomfort from her implants being under the muscle. She was looking for physical relief and a more natural looking result.  

She also had a strange double areola tattoo from past attempts and nipple and areola reconstruction that she hated. The plan for her revision was to convert her submuscular implants to a prepectoral position, change her older silicone implants for the newest devices, and remove the excess tattoo pigment on the sides of her breasts.

Her pectoralis major muscles were returned to the chest wall. New smooth round silicone gel breast implants were placed on top of the muscle. Her areolas were made to look more normal. We could not talk her into liposuction of the axillary rolls (which is routine in our office) or redo tattoo of the areola circle to make it look more natural.  

Follow up photos are shown 5 weeks after surgery. She feels much more comfortable physically and she looks better too!
