Before and after breast revision in a 30 year old woman. She had a previous breast augmentation and an original tubular breast shape. Her first surgery had disrupted her right inframammary fold and created a “double bubble” deformity, where the breast implant slid below her fold, creating this deformity and discomfort from the implant sitting on her rib.
A double bubble occurs as a consequence from injury to the inframammary fold from dissection too low when creating the breast pocket. It is important to realize and respect the anatomy and boundaries of the breast in any surgery, whether reconstructive or cosmetic.
This patient had also developed a hypertrophic scar under her right breast that was not addressed by her past surgeon. Her revision involved reconstruction of her right inframammary fold and scar revisions under both breasts. A small amount of fat was also directly excised under her right breast as part of the reconstruction to accentuate the fold.
She required aggressive scar therapy and several steroid injections after surgery as she was prone to developing hypertrophic scars. Follow up photos are shown 6 months after surgery.
