Before and after bilateral breast revision in a 44 year old breast cancer survivor. She previously had a breast reduction and later was diagnosed with breast cancer, requiring bilateral mastectomies.
Her original breast reconstruction involved stiff, shaped textured breast implants that were put under the muscle. She had unusual puckering from acellular dermal matrix (ADM) placed to “try to help things heal better”. Her animation deformity was awful. She had actually considered having her implants removed altogether.
Her revision involved removal of her old textured implants, replacement of her pectoralis major muscles to the chest wall and total removal of the ADM which had not incorporated and was actually creating much of the deformity of the lower pole of her breast.
Next, new intermediate stiffness cohesive silicone gel breast implants were placed on top of the muscles. This is also called “pre-pectoral” reconstruction by some academics – however, this technique has been performed in my practice since I started in 2006!
Her nipples and areolas were lifted and liposuction removed stubborn fat from her armpit region and back fat/bra rolls. Follow up photos are shown 8 months after surgery. She now exercises without her breasts jumping, and she is finally able to move forward after being diagnosed with breast cancer at a young age.
