Before and after bilateral mastopexy and augmentation in a 33-year-old woman. After having children, her breasts lost most of their volume. She also had asymmetrical breast size, with her right breast being larger than the left. She was looking to regain the former fullness of her breasts but to maintain a natural look.

A combined breast augmentation and slight lifting of the nipples using the peri-areolar mastopexy technique was planned. Smooth round silicone gel breast implants were placed subglandularly on top of the muscle. A larger implant was placed on the left side to achieve improved symmetry. To lift the nipples, an incision was made around the areolas, but the lower pole was left alone to keep a full and round lower pole.

Follow up photos are shown 6 weeks after surgery. She is right-handed; therefore, her right dominant side is more swollen, as expected. Her breasts still have a slight flat appearance from the sides due to postoperative edema, and a starburst pattern of swelling around her areolas which will resolve over the next several months.
