Before and after bilateral nipple-sparing mastectomies and immediate implant reconstruction in a 45-year-old woman. She previously had two lumpectomies on the left side which had positive margins each time, so she required a completion mastectomy. She elected to have a prophylactic mastectomy on the right as well.

Her left lumpectomies had been done through an incision in the lower pole of her breast, giving her the effect of a breast lift. At the time of her mastectomies, a vertical incision was made to access the right breast as well in order to also reduce the lower pole skin of this breast as well.

Smooth round permanent postoperatively adjustable implants were placed on top of the muscle, in the prepectoral position, where the breast tissue was removed. Her drains were in place for ten days. After surgery, she began implant massage and returned to work and exercise in six weeks.

Follow up photos are shown 6 months after surgery. Her right breast is still settling because of the additional incision in the right lower pole; she will achieve her final results at one year. She is considering keeping her saline implants in place and just having her implant ports removed in the office so that she can spend as much time as she can with her children during their formative childhood years.
