Before and after bilateral nipple-sparing mastectomies and implant reconstruction with a mastopexy in a 52-year-old woman with right cancer. She chose to have a mastectomy on both sides and prepectoral implant reconstruction. Her goal was to have breast reconstruction as least a full as her natural breasts, if not fuller and lifted.
During the same procedure as her mastectomies, smooth round postoperatively adjustable saline-filled breast implants were placed on top of the muscle, in the space where her breast previously was. Her nipples and areolas were saved as free grafts and when the skin was closed to achieve a breast lift, they were reapplied higher up on the breast. After surgery, her implants were inflated to their final size within three weeks.
She completed chemotherapy and recovered fully for nine months before her final stage, which was implant exchange to smooth round silicone gel and areola tattoo to brighten their color – since free nipple and areola grafts often lose some of their pigment during healing.
Follow up photos are shown 4 years after her mastectomies, reconstruction and breast lift.
