Before and after bilateral nipple sparing mastectomies and immediate breast reconstruction in a 38 year old woman.  She had previous implants placed in the submuscular position.  She wished to be reconstructed to a similar size as her preoperative augmented breast size.

Follow up photos are shown at one month after surgery.  Smooth round adjustable saline breast implants were placed in the space where her breast tissue was – this is known as the subcutaneous or “pre-pectoral” plane.  No other products like acellular dermal matrix (ADM) or mesh was used.  The incisions are hidden in the fold under her breast.

A small circular band aid is visible over the remote implant port.  Her implants were inflated 60% of the way at the time of her mastectomy and two additional fills in the office at 2 weeks and 3 weeks completed the inflation.  There was no need for “expansion” of her skin – it was simply filled out with the implant.

She will continue to heal and undergo removal of her implant ports after three months.  She has the option of exchanging her saline implants to silicone at the time of port removal.  However, switching the implants are not necessary, as these are permanent implants.
