Before and after bilateral explantation in a 45-year-old woman who had saline breast implants inserted nearly 20 years previously. She no longer wanted her implants and wished to minimize the amount of surgery she needed. While she was happy to learn about saline removal via implant deflation as an interim option, she elected to plan to have the entire breast implant removed as her procedure of choice.

Her explantation surgery was planned under local anesthesia, taking just over one hour to perform. She was given an oral anti-anxiety medication and offered laughing gas during the procedure. The existing incision hidden in the fold under her breast was used to access the implant space. Her implants were removed, and the pocket was inspected and found to be quiet and devoid of any acute issues such as inflammation or capsular contracture. Because of this, no capsulectomy was required.

After surgery, she showered in two days and resumed exercise in two weeks. Early follow-up photos are shown just 4 days after surgery. She loves her natural breasts!
