Before and after bilateral explantation in a 67-year-old woman with contracted, old silicone implants that were inserted over thirty years previously. She wanted to minimize her surgical recovery and avoid general anesthesia if she could.

Both breast implants were removed in the office under local anesthesia. Upon opening the implant space, ruptured implants were encountered – no surprise, since her second-generation implants had been in place since the 1980’s. All foreign material was removed, and a full washout of the implant pockets was performed prior to closing the incisions under her breasts. She declined the option to have liposuction of her axillary rolls (armpit fat).

Long-term follow up photos are shown one year after surgery. She loves her smaller, natural looking breasts and that her procedure was so straightforward!
