60-year-old woman with a history of left breast cancer, implant reconstruction, chemotherapy and radiation. She wished to convert her implant reconstruction to a flap. Her left breast implant that was too small and misshapen was removed and replaced with a DIEP flap. For balance and a slightly fuller but natural result, a small right breast implant was placed. Postoperative images after a left areola tattoo with bright pigment that will fade over months are shown.
