46-year-old woman with a history of left breast cancer who was not initially offered reconstruction. Am SIEA flap reconstructed her left breast, and a right balancing breast reduction gave her smaller, shapelier breasts and symmetry, together with a smoother and flatter abdominal contour. Postoperative images at one year after her flap with mature scars and a reconstructed nipple and areola are shown.
