Before and after bilateral mastectomies and immediate breast reconstruction with the DIEP flap in a 47 year old mother of two. She had had a past breast reduction. A prophylactic nipple-sparing mastectomy was performed on the left and a skin-sparing mastectomy on the right for a right-sided breast cancer.
Bilateral breast reconstruction was performed using the deep inferior epigastric artery perforator flap. A second-stage procedure at one year reconstructed her right nipple by way of a free nipple graft (nipple sharing from the left side) with a tattoo performed bilaterally for equal color intensity and the best symmetry of her nipples and areolas.
Free fat grafting complemented the revision, and her abdomen, hips and thighs were sculpted at the same time. The “back fat/bra roll” area was also debunked as a revision of her breast reconstruction using liposuction during this revision.
Follow up photos are shown at 5 years postoperatively. This patient was happy to think about her breast reconstruction as her “Mommy Makeover”. She loves her new body and has moved on with her life!
