Before and after left non-nipple-sparing mastectomy and DIEP flap reconstruction in a 60-year-old woman with breast cancer. Her aesthetic goal was to be fuller than her preoperative breast volume. Therefore, a balancing breast augmentation was planned at the same time for the right breast.
For the left breast, lower abdominal skin and fat were microsurgically transplanted to the chest on the deep inferior epigastric artery perforators (DIEP). The patient unfortunately experienced poor healing in the lower aspect of the left breast, likely due to immunocompromise from neoadjuvant chemotherapy. It gradually healed over several months of dressing changes.
Six months later, her left nipple was reconstructed using the local flap technique. Medical tattoo created a new areolar circle on the left. The right areola was also tattooed for symmetry. Follow up photos are shown 8 months after her breast reconstruction and two months after nipple-areola reconstruction.
