Before and after bilateral breast implant exchange and mastopexy in a 38-year-old woman. Her previously placed saline breast implants were under the muscle. She had delivered and breastfed three children since having her breast augmentation and developed droop of her breasts and a long-breasted, matronly look. She wanted to keep the fullness of her breasts but have a more natural look and to be slightly smaller.

Her revision included removal of the saline breast implants, replacement of her pectoralis major muscles to the chest wall, and insertion of smaller smooth round silicone gel breast implants on top of the muscle, in the subglandular position. Her nipples and areolas were lifted in a skin-only mastopexy, allowing contouring of the breast skin around the implants.

Follow up photos are shown 6 weeks after surgery. She is ready to start scar therapy and resume full activities.
